It was observed one staff member had two infants that are 13 months old, and a 4 year old child in the main room, more than 18 months apart. Based of the ages staff member was out of ratio.

It was observed that one infant sitting in a high chair did not have safety straps buckled.

At the time of inspection, the last fire inspection documented was 09/11/2024.

No documentation for the smoke detector, carbon monoxide and fire extinguishers was provided on form 7263.

The school age children were observed eating their snack without washing hands.

A double outlet was observed without childproof covers and it was not a safety outlet.
