On 02/02/2026, a child in care at the operation was left alone by a caregiver in a classroom. The child was found alone in a classroom by the child's caregiver and the child's mother when the child's mother arrived to pick the child up from the operation at the end of the day.

746.305(a)(6) - Report Situation Placing Children at Risk

On 02/02/2026, a child was found by a caregiver and the child's parent alone and unsupervised in a classroom at the operation. This incident was reported to Child Care Regulation (CCR) by the child's parent on 02/13/2026; however, the operation did not report the incident to CCR.

On 02/02/2026, a child was found by a caregiver and the child's parent alone and unsupervised in a classroom at the operation. This incident was reported to Child Care Regulation (CCR) by the child's parent on 02/13/2026; however, the operation has not yet provided CCR with an Incident or Illness Report Form 7239 detailing the incident.

At time of inspection, the unitary surfacing on playground has not been maintained. There are missing tiles and some tiles have been lifted and moved.
