In the older infant classroom, after a diaper change the child's hands were not washed. NOTE: This was corrected when another caregiver came in to assist.

The annual health inspection was last conducted on 12.19.24.

At the time of the inspection the liability insurance available expired 1.29.25.

746.603(a)(3) - Children's Records - Health Statement

Four of the ten children were missing healthcare statements. Three of the ten children files were missing immunization records.

746.605(6) - Required Admission Information - Emergency Contact

One of the ten children was missing parent's name, telephone number. One of the ten children was missing release information. Two of the ten children were missing field trip permission. Two of the ten children were missing special care statement.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

During the course of the investigation, one out of the four caregivers did not have an updated CPR/First Aid certification.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Operation video footage demonstrated an operation staff member grab a child twice while redirecting.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

During the course of the investigation, it was verified a staff member will yell at children when redirecting them.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

A staff member failed to use good judgment when as a consequence of behavior, she forced a child to pick up toilet paper with her bare hands from the restroom.
