During the follow up inspection, the school-age playground and the Pre-K playground still did not have an adeqaute level of loose fill.

During the inspection, an infant child was observed eating small pieces of food directly off of the the table surface. No plate, napkin or other food holding device was present with the food.

During the inspection, every desginated play area surveyed had two inches or less of loose fill, with bare ground showing in many places.

During the inspection, the operation had no manufacturers instrctions for the the operation of the splahpad, making it impossible to ascertain if proper maintance per manufacturer's instructions had been occuring.

During the inspection, the outlets in a room being used by a class of four-year-old children were not all covered.

Based on information provided in a DFPS investigation, it was determined that the nap room was not arranged in a manner that supported the caregiver to be able to intervene as necessary to ensure safety.
