Operation completed but did not have documentation showing a fire drill was done in December 2025. This was corrected at inspection when a caregiver documented a on an emergency practices form showing a fired drill was completed 12/18/2025.

Operation does not have current liability insurance policy availabel for review. Insurance expired 05/09/2025.

746.4503(b) - Cots, Beds, Mats Labeled With Child's Name

In the afterschool class, cots were numbered with no matching list that showed which child was napping on wich cot. This was corrected during inspection when caregiver created a list matching each numbered cot to a child by name.

746.4003(a)(7) - First Aid Kit Incomplete - Thermometer, preferably non-glass

Operation does not have a working thermometer available during inspection. Thermometer needs batteries.

746.5619 - Plan to Handle Transportation Emergencies

Operation does not have transportation emergency plan available for review.

746.801(18) - Required Records Maintained and Made Available - Children's Product Certification

Operation did not have documentation available for review showing the consumer product safety commission website was checked since February 2024. This was corrected at inspection when a caregiver reviewed the website and completed form 2885, dating it 4/24/25.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2024 Report with a run date of 01/17/2025 indicates that the operation did not report the number of employees who left employment in 2024 by the required date of 1/15/2025.

746.5315 - Smoke-Detection System Tested

Operation has an electronic smoke alarm system that has not been inspected since 07/17/2023. A new fire inspection was due 07/17/24.

746.5101(b) - Annual Fire Inspection - Conducted by Fire Marshal

Operation does not have a current fire inspection. Last inspection completed on 07/17/23.

746.1309(e) - Documented Annual Training- Required Content

In review of records, three staff members do not have proof of training in Poison Prevention/Hazardous Material, food allergy or administering medication.
