The operation failed to notify parent regarding a safety issued within the center after a clogged restroom causing flooding in the facility. The lack of timely communication did not ensure that parents were adequately informed about the environmental safety concern effecting the center.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A total of two teachers and ten children were present inside the classroom. One operational staff were observed taking care of children during naptime inside the classroom without a clear background check. The second teacher had a clear background check and age of the children are two year old.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A child was left unsupervised inside the toddler classroom for thirty-seconds.

746.501(a)(20) - Written Operational Policies - Parent Rights

Ten children files were review. Ten out of ten children files did not have did parental rights form 2987 for licensing to review.

746.1111 - Education Outside of U.S.

A total of ten operational staff files were review. One out of ten operational staff files did not have their HS diploma translate to English to established their her educational credentials are equivalent to HS diploma program in the United States.

746.303(b) - Report Number of Employees

The operation is being cited by assessment because the operation failed to report the number of employees left the childcare center by the previous calendar year 2024.
