During the monitoring visit, there were there were three bottles and two sippy cups not labeled in the infant classroom. The director entered the classroom and labeled the bottles with a Sharpie with the infant's first name.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

During the visit, there was one caregiver who did not have a First Aid certificate available for review.

746.3407 - Maintenance of Building, Grounds and Equipment

During the visit, the playground equipment toy was missing a bolt to secure it to the ground. The equipment is located on the playground.

Two out of ten children did not have updated shots available for review.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

During the visit, there was one caregiver who did not have a CPR certificate available for review.

This standard was evaluated regarding an investigation, and a violation was found. Through interviews and observations, it was determined that the caregiver used inappropriate language by yelling at young children in care at the operation.

This standard was evaluated regarding an investigation, and a violation was found. It was determined that the caregiver has physically disciplined their own child while in care at the operation.

746.2207(c)(4) - Screen Time Activities - Not Used During Eating or Rest Times

During the follow-up visit, I observed the young children eating breakfast while the television was on in the eating area of the operation. The director turned the television off upon request.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

During the visit, the mulch was below level on the outdoor playground.

746.2805(6) - Prohibited Punishments - Using Harsh, Abusive or Profane Language

This standard was evaluated with regard to an investigation, and a violation was found. The caregivers used profanity and yelling when communicating with young children in care.

746.521(1) - Parent right to enter and examine operation without advance notice

This standard was evaluated with regard to an investigation and a violation was found. It was determined that parents are allowed to enter the building to sign children in and out of the operation. A caregiver or director will bring the children to the front or take the children to the back in the area where the classrooms are located. Parents do not have access to enter classrooms at the operation.

One child did not have an updated feeding form. The last form available for review was from December 2024.

Three out of ten children did not have up-to-date shot records available for review during the monitor visit.

The operation had a fire inspection conducted on 10/2023 available for review during the monitoring visit.

Two out of ten staff did not have the required annual trainings for shaken baby syndrome, prevention of SIDS, understanding early childhood brain development, emergency preparedness, communicable disease, preventing and controlling food allergies, building premises, and handling and storing certificates available for review during the monitoring visit.
