746.603(a)(2) - Children's Records - Admission Information

It was determined during an investigation that a child was allowed to attend the operation without having the required admission information.

Six of ten staff files reviewed where lacking their date of hire, notarized affidavit, or copy of their ID.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

Two of ten staff were lacking verification of completion of their CPR and First Aid Training.

It was observed throughout the operation that air vents were dirty, there was peeling paint on several walls and fray carpet was present on a window seal.

Six of ten children's files were lacking complete address for the child, complete address for their emergency contact, parent signatures that parents rights or operational policies were received, date of admission or emergency authorization for emergency care.

746.3701 - Safety - Areas Free From Hazards

It was observed that a metal piece of equipment had paint that was chipping exposing metal that rust can cause injuries and exposures to harmful materials.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

This standard was reviewed as part of an investigation and found non-compliant based on information obtained. Two of four infants written feeding instructions had not been updated in the past 30 days. One child was last updated in January and the other child was last updated in February.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

This standard was reviewed as part of an investigation and found non-compliant based on information obtained. It was discovered that a caregiver was not following feeding instructions for a child when they feed the child cereal and baby food prior to parent giving permission.

746.305(a)(6) - Report Situation Placing Children at Risk

During the investigation it was determined that the center failed to report an incident that a staff had allegedly inappropriately picked a child up in a timely manner.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was reviewed as part of an investigation and found non-compliant. It was discovered that a child had wandered out the front door unsupervised.
