All caregivers did not have all 24 hours of annual training.

A child was out on the porch riding a trike without supervision for an unknown length of time.

Four employees hired after September 2023 do not have preemployment affidavits in there files.

746.605(1) - Required Admission Information - Name and Birthdate

Two children do not have admission record in their files. One does not have all required information and the other does not have an admission record at all.

746.613(a) - Required Immunizations

Six children out of 10 children's files evaluated, do not have immunization records in their files.

746.901 - Required Personnel Records

Four employees were missing several required record documents in their files. Notarized affidavit, preemployement affidavits, backgrpund check letters.
