Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support that an eight-year-old child was left in a vehicle unattended.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

During the DFPS Investigation, it was found a child was left unsupervised on the van. The caregiver at the time did not do a face to name check when all the children left the bus.

746.1201(4) - AP Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited

This standard was found deficient as part of a DFPS Investigation. There is sufficient evidence to support the allegation of neglectful supervision. A caregiver did not check the van after transporting, which resulted in an eight-year-old child being left in a vehicle unattended for approximately 13 minutes.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
