746.2805(3) - Prohibited Punishments - Grabbing and Pulling

A child was aggressively grabbed/pulled by the wrist/arm during a disciplinary incident at the facility. The child was uninjured as a result of the incident. The caregiver responsible was terminated for her actions. Current operation staff have received additional trainings on positive discipline/guidance to mitigate risk.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two out of four infant feeding instructions were expired. The Director is going to correct this by getting the feeding instructions updated.

746.3407(8) - Garbage Inaccessible to Children; Maintained in Sanitary Manner

There were two trash cans in the 3-4 year old room that did not have lids. The trash cans contained food items in them. The Director corrected this at the time of the inspection by replacing the trash cans with cans that have lids.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

The Director and the van driver both had expired transportation training. The Director is going to correct this by getting the transportation training done.

746.1203(4) - Provide care consistent with child's habits, interests, strengths, special needs, including any special supervision needs

This standard was evaluated regarding a report received and found to be deficient. Evidence received through interviews supports that a caregiver did not properly supervise a child resulting in the child exiting the playground, going to the road and being found by a passerby.
