Based on information obtained, there is sufficient evident to support that two toddlers were unsupervised in the restroom. While unsupervised, they opened the door, which resulted in an injury to another child.

Based on information obtained, there is sufficient evidence to support a caregiver did not sanitize the diaper changing surface while changing multiple children.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

One caregiver was responsible for 15 children between the ages of 14 to 20 months during naptime . The specified age group was 18 months. The maximum number of children for this specified age group is 9 children. The classroom was over ratio by 6 children.
