This standard was found deficient as part of a DFPS investigation. The operation failed to report an incident of a child injured by a caregiver with in the 2 day required timeframe to Child Care Regulation.

This standard was found deficient as part of a DFPS investigation. There was sufficient evidence to support that the caregiver grabbed a child by the arm and pulled the child placing them in the chair forcefully.

This standard was found deficient in relation to the DFPS investigation. The director failed to ensure that a caregiver that was previously cited for inappropriate discipline was following safe practices and disciplining the children correctly.
