Observed 5 infants asleep between the ages of 4 month to 12 months alseep in a restrictive device (swing and bouncer) covered with blankets at the time of inspection.

Observed 2 staff supervising a group of children while using their personal cell phone at the time of inspection.

746.5617(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

Fire extinguisher in one transportation vehice was not secured as it was observed on the side of the door in the dirvers side at the time of inspection.

First aid kits in both transportation vehicles did not have thermometer at the time of inspection.

Infant classroom was moved to a different classroom without notifying licensing.

The bottles in the infant room were not labeled with the children's first names and last initials.

I observed 4 feeding instruction sheets. Two were not dated. One was dated 09/27/2024 another was dated 10/04/2024.
