This standard was evaluated regarding an investigation, and a violation was found. The operation did not notify the childcare licensing regulations regarding the victim being left in the vehicle at the operation.

746.5605(3) - Safety Precautions for Loading and Unloading Children - Account for All Children

This standard was evaluated regarding an investigation, and a violation was found. The victim was not identified by the driver for entering the vehicle or getting off the vehicle based on the driver's student list of names being transported to the operation from the elementary campus.

746.5605(4) - AP Safety Precautions for Loading and Unloading Children - No Child Unattended

This standard was evaluated regarding an investigation, and a violation was found. The victim was left in the operation vehicle unattended. The victim exited the vehicle through the emergency door open independently.

746.3417(1) - Children Handwashing Before Eating

During the investigation visit, I observed children entering the eating area without washing their hands before being served breakfast. I observed the children sit down and eat their breakfast without handwashing or using hand sanitizer.

746.3503(d) - Diaper Changing Equipment - Hand-washing Sink in Diaper Changing Area

During the monitoring visit, the diaper changing table was outside of the infant classroom door.

During the monitoring the visit, the cribs were place side by side in the infant classroom.

One child did not have the medication authorization form signed by the parent. I reviewed the medication at the operation.

The operation did not have the duration of outdoor time in the operational handbook.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

One infant did not have feeding plans available for review during the monitoring visit.

746.5617(a)(1) - Required Equipment in Vehicles - List of Children

This standard was evaluated with regard to an investigation, and a violation was found. The operation did not have the victim's name listed on the sign-in sheet for pickup from the after-school program.

746.2701(4) - Care Requirements for School-age - Caregiver Aware of Arrival/Departure

When the elementary school sent a child to on site after school program by an error, the driver failed to confirm with the parent or the daycare center, if the child needed to be picked up by the daycare.
