Food items served during PM snack and supper do not match the menu for the day.

Transportation binder is missing the children's emergency medical transport and treatment authorization forms.

Most recent trainings records for three out of four staff records reviewed were not available at the time of inspection. Immunization records for three out of six children's files reviewed were not available for review at the time of inspection.
