A caregiver was present in the infant room without an initial approved background check. This was corrected at the inspection as the caregiver was sent home during the inspection.

Caregiver did not complete a head count during transition periods.

A PreK child was left in the classroom unattended.

There was one smoke detector in the kitchen that was not in working order.

There is enough evidence to indicate that the operation was in violation of this minimum standard. The caregiver was in the bathroom leaving the children alone in the classroom.

746.2803(4)(D) - Positive Methods of Discipline and Guidance - Brief Supervised Separation

This operation is not in compliance with this standard. The caregiver acknowledged and video evidence shows that the child was in timeout longer than the required five minutes.

746.611(a)(1) - Required Health Statement - Written, dated within past year, indicates child is physically able to participate in operation's child-care program

One child was missing their health statement in their files to be reviewed by CCR.

746.2805(1) - Prohibited Punishments - Corporal Punishment

During the investigation, it was determined that a caregiver spanked children in care on their bottoms using her hand.
