During the investigation, it was confirmed that a child ran out of the back door of the classroom that led to the playground, leaving a parent in the classroom with the remaining children.

One out of four files evaluated did not have the current immunization record available for review. Note: This issue was corrected during the inspection when the director was able to obtain the updated immunization record.

One of four files evaluated did not have the admission date on the child's enrollment form. Note: This was corrected at inspection when the date was entered by the director.

The director and a caregiver did not have current training in SIDS, Shaken Baby Syndrome, and Early Childhood Brain Development.
