A caregiver, who previously worked at the center, was present in the classroom without an active background check. Note: This was corrected at inspection when the caregiver was removed from the classroom and her background check was resubmitted.

746.1309(a) - Documented Annual Training Area - 24 Hours Required

One out of the ten staff files reviewed did not have the total required 24 hours of required trainings available from the last full employment year.

Based on the DFPS investigation it was determined that a child was grabbed with sufficient force to leave a bruise.

During a DFPS investigation, the director only provided sections of video footage for review instead of providing the entire recording.

Based on a DFPS investigation, video footage showed a caregiver left a group of children unsupervised for less than one minute when she exited the classroom.

This standard was found deficient as part of the DFPS investigation.

Based on a DFPS investigation, it was determined that a caregiver did not share information about a child's injury with caregiver in the next shift.

A child's Food Allergy Emergency Care Plan lacked doctor's or health professional signature.
