Two infants were asleep on non-sleeping equipment. One infant was asleep on a bouncers. The other infant was asleep on boppy. The infant on the boppy was moved from the boppy to the crib once the inspector brought it to the caregivers attention.

Based on the preponderance of evidence gathered, there is sufficient evidence to determine the parent handbook (operational policies) did not include a policy on vaccine preventable disease for employees.

There were 17 individuals not listed on the employee/staff list who had an active background check and are no longer associated with the operation. The background check had not been inactivate inactivated timely.

746.4813(c) - Measurement of Use Zone for Rotating and Rocking Equipment - No Overlap

The use zone for a rocking outdoor play equipment overlapped with the use zone of the slide in the outdoor play area one. The play equipment was removed from use.

Three infants lacked the 30 day updated written instructions sheets signed by a parent. The last ones in file were June and July.

One child's record lacked an immunization record.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

At inspector's arrival to a school age classroom, a caregiver had 28 children in care between 8-11 years old. Another child arrived which brought the child/caregiver ratio to 29 children to one caregiver. This was corrected at the inspection when the director moved 3 children to another classroom, and had a staff enter the classroom to assist.
