A caregiver positioned herself on the floor, in a corner, behind a shelf, with an obstructed view of the class during nap. The caregiver remained seated and was unable to supervise the entire class.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Video evidence shows a caregiver retrieve a personal electronic device and keep it out and, in her possession, while caring for children during nap.

746.2803(4) - Discipline - Positive Methods of Discipline and Guidance

During the course of the investigation, it was determined that a caregiver failed to use positive methods of discipline and guidance to redirect children's behavior when she told the children the police would come and take them away if they did not lower their voices.

746.3417(3) - Children Handwashing After Toileting or Diapering

During the course of the investigation, it was determined that eight children had their diapers changed without having their hands washed afterwards.

During the course of the investigation, it was determined that the incident occurred on 11/4/2024 and was reported to the center management that afternoon. The parent of the child was notified the morning of 11/5/2024.
