A child who had food allergies listed on their admission paperwork at enrollment on 4/1/24 did not have a completed food allergy emergency plan signed by a parent and a healthcare professional as required. A food allergy plan was not completed until after the child had an anaphylaxis reaction at the operation on 5/17/24.
