A caregiver was observed changing 2 diapers. The caregiver wore a glove one one hand. The caregiver did not remove the glove after either diaper change and did not wash their hands in between the diaper changes.

An 11 month old infant was observed to be sleep in a ctib with a pacificer clipped to them. A 12 month old was observed to be sleeping with a stuffed animal.

Crinbs in the infant room were not observed to have snug fitted sheets.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

Upon arrival, an 11 month old was observed to be sleep with several toys in the cdrib. NOTE: The child was asleep for the duration of the inspection. The toys were observed in the crib for 15 mintues before being removed by the caregiver.

A group of children was observed to be watching Paw Patrol on tv. The show was not related to an activity.

A 2yr old was observed to be sleeping with a training cup.

A 7yr old child was observed to be unservised in a kitchen/cafeteria area. The child was observed climbing on a sink to plug something into an outlet located behind the refrigerator.

Based on DFPS investigation there is enough evidence to validate that a caregiver did not intervene to keep one child from hitting another child.

746.2805(8) - Prohibited Punishments - Restrictive device for time out

Based on DFPS investigation there is enough evidence to validate that a six-year-old child was placed in a highchair as a form of discipline.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

Based on DFPS investigation there is enough evidence to validate that a caregiver humiliated a child when a six-year-old child was not treated with respect due to being placed in a highchair for discipline.

746.303(b) - Report Number of Employees

All Licensed childcare centers must annually report to CCR the number of employees who left employment the previous year by January 15. The deadline for entering the number of employees who left employment in 2023 is 1/15/2024. If you have not already you must log in to your childcare regulation account and report the number of employee that left your employment in 2023

Notification of field trip was not posted in a prominent place were parents and others may view it. This notice must remain posted until all children on the field trip have returned to the daycare.
