One child file did not have a current immunization record on file.

746.4605(a)(3) - Equipment Prohibited for Younger Than 5 Yrs - Fulcrum Seesaws

Based on the evidence gathered, a two-year-old child was allowed to play on a fulcrum seesaw while in care, which resulted in a minor injury. This was corrected at inspection when the fulcrum seesaw was removed from the playground and made inaccessible to children younger than 5 years of age.

746.5607(e) - Safety restraint devices for child used when vehicle is on and during all time vehicle in motion

Based on the evidence gathered, at least three children who require a child safety seat or booster seat while being transported, were not secured in the proper safety restraint device during the vehicle accident.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk

Based on the evidence gathered, the operation did not immediately notify parents that their children were involved in a vehicle accident. The operation waited to inform parents at pick up later that day. One parent wasn't notified until the next day.
