746.2805(3) - Prohibited Punishments - Grabbing and Pulling

A caregiver was observed grabbing a child by the arm while taking him back to the room.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

A caregiver was witnessed on her cell phone at the time of inspection.

746.1203(5) - Responsibilities of Caregivers - Interact positively with children

This standard was evaluated in regard to a report and found deficient. A caregiver was witnessed on video throwing a toy at a child. The caregiver also threw a ball at a child and they fell down when hit with by the ball.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

This standard was evaluated in regard to a report and found deficient. A child was left unsupervised on the playground, during a transition from recess.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

This standard was evaluated in regard to a report and found deficient. Caregivers failed to properly follow policies and procedures for accounting for children, during a transition and a child was left unsupervised.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A caregiver left a child in the building alone unsupervised when she left the building and went outside.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

This standard was evaluated in regards to a report and found deficient. A caregiver failed to utilize good judgement when she left a child unsupervised alone in the building.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

During the monitoring visit, one caregiver did not have a CPR/First Aid card available for review.

746.3401(a) - Annual Sanitation Inspection

The operation's last sanitation health inspection was October 3, 2023.

The operation did not provide an incident report to the parent in a timely manner that is required by the minimum standard. This standard was evaluated with regard to an investigation, and a violation was found.
