A caregiver was observed in the video footage to be inappropriately grabbing a child.

746.303(b) - Report Number of Employees

A review of the Employees Who Left Employment in 2025 data revealed that the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Five infants were observed to be seating in high chairs during circle time. Caregivers were asked how long the infants had been seating in the highchairs for circle time and the caregivers stated for thirty minutes.

746.2805(5) - Prohibited Punishments - Humiliating, Rejecting, Yelling

During the inspection, a caregiver was heard yelling at child that was not following directions in the classroom from the operation's front office.

746.3415(4) - Employee Handwashing After Diapering a Child

Two teachers in two different classrooms was observed to not wash their hands after diaper change.

746.3417(3) - Children Handwashing After Toileting or Diapering

In two different classroom toddlers' hands were observed to not be washed after a diaper change.

746.3407 - Maintenance of Building, Grounds and Equipment

During the walk through, three rooms were observed to have play materials such as baskets that were observed to be damaged. Corrected at Inspection: During the inspection, the damage baskets were removed from the three classrooms.

746.3809 - Medication No Longer Used

Eight medications for different children were observed to be expired.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Based on the information obtained, there is sufficient evidence to suggest that a caregiver did not demonstrate competency, good judgment, and self-control in the presence of children by pulling a child?s hair.

Based on the information obtained, there is sufficient evidence to suggest that a caregiver did pull a child?s hair because the child was not following directions.
