This standard was evaluated and found deficient. The staff was observed from the video footage using her cellphone while supervising a child.

This standard was evaluated and found deficient. A child was observed sat by the staff in a restrictive device for more than 10 minutes.

Medication was administered to a children before written authorization was provided from a parent.

The person who administered medication to a child, did not have training in administering medication to a child in care.

It was observed that one infant was propping a bottle. The child can't hold the bottle yet.

A caregiver was observed on camera forcing a child to eat.
