The following safety hazards were found on the playground: Three holes at the bottom of fence line and a section of the fence line that is falling down into the playground area. Metal/wooden bench has rusted impalement hazards and loose board. Interior crack at top of slide on the large boat play structure. Garden hose loose on sidewalk leading to and from playground.

746.5607(b) - Child Passenger Safety Seat System - Not expired or damaged in accident

Booster seats in four of the operation's six vehicles used for transportation were expired. Booster seats had expiration dates of 2014 and 2015. The operation purchased and installed brand new booster seats during this inspection. The operation brought the deficiency into compliance at inspection.
