Based on information gathered during the DFPS investigation evidence supports that two caregivers can be seen using their phone while in the classroom with children in care.

746.3415(1) - Employee Handwashing Before Eating or Handling Food

Based on information gathered during the DFPS investigation evidence supports that a caregiver made a bottle for an infant without washing their hands after changing a child?s diaper.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

Based on information gathered during the DFPS investigation evidence supports that infants were sitting in restrictive devices for longer than 15 minutes and they were not eating.

Based on information gathered during the DFPS investigation evidence supports that a caregiver did not wash their hands after changing an infant?s diaper.

Based on information gathered during the DFPS investigation, evidence supports a caregiver aggressively grabbing and picking up an infant by both arms resulting in injury that required medical attention.

Based on information gathered during the DFPS investigation, evidence supports that a caregiver did not use good judgement or self-control when roughly placing an infant in the Sit Me Up Chair causing unsupported head to hit front part of chair. A caregiver did not use good judgement while picking up pacifiers off of the floor and placing them back into an infant?s mouth without being sanitized. Caregivers did not use good judgement and notify the director, or the parent, when a caregiver admitted to picking up an infant and hearing his arm pop.

746.307(b)(1) - AP Parental Communication - Injury Requires Treatment by Health Care Professional or Hospitalization

Based on the information gathered during the DFPS investigation the parent was not notified about an injury to an infant that required medical attention.

746.2417(3) - Activities for Infants - Physical Activity including Supervised Tummy Time and Safe, Clean, Uncluttered Area

Based on information gathered during the DFPS investigation evidence supports that infants are not being directly supervised during tummy time. It can be seen in the video that the infants are placed on the floor, on their tummy, and caregivers walk away from them.

746.3505(e) - Diapering Procedures - Sanitizing Diaper Changing Area

Based on information gathered during the DFPS investigation evidence supports that a caregiver did not sanitize the changing table after changing an infant?s diaper.

Based on information gathered during the DFPS investigation evidence supports that an infant was observed sleeping in a Sit Me Up chair for more than an hour.

746.3501(2) - Diaper Changing Steps- Cleanse Child with Individual Cloths

Based on information gathered during the DFPS investigation evidence supports that a caregiver did not cleanse an infant with an individual cloth or disposable wipe after a soiled diaper was removed during a diaper change:

loose staples and sand were located under and around the diaper changing mat in the two year old room and the 12 to 18 month room. This was corrected at inspection and the mats and tables were cleaned.

In the infant room one Infant Care Instruction sheet for was 4 days past due to be updated. The form was last signed on 7-23-25 and is required to be updated every 30 days.

746.303(b) - Report Number of Employees

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.
