Based on the evidence gathered during the investigation, it was observed in a video that three caregivers were using their personal phones while caring for children.

Based on the evidence gathered during the investigation, it was observed in a video that the caregiver gave the children a snack, cookies, placing them on the table without any plates or napkins.

746.3417(3) - Children Handwashing After Toileting or Diapering

Based on the evidence gathered during the investigation, it was observed in a video a caregiver changing diapers but did not wash any of the children's hands after the diaper change.

746.2205(a)(4) - Written Activity Plan - No Long Waiting Periods

Based on the evidence gathered during the investigation, it was observed in a video that children were placed in a feeding table for 30 minutes as a waiting period while caregiver filled out forms.

746.2803(4) - Discipline - Positive Methods of Discipline and Guidance

Based on the preponderance evidence a caregiver restrained a child between a table and herself as a method of guidance and redirection of children. This method of guidance does not encourage self-control.

Based on the evidence gathered during the investigation, a child was grabbed and pulled up forcibly by a caregiver as a method of discipline and guidance.

Four out five employee records reviewed for their annual year were missing the required trainings hazardous materials, building safety, allergic reactions, medication administration emergency preparedness, and communicable diseases.
