The electrical outlet in the toddler room was uncovered. Note: The caregiver covered the outlet before I left the classroom.

746.801(3) - Required Records Maintained and Made Available - Personnel and Training Records

Staff files were not available during my inspection due to the person in charge not having access to the office.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

There were no feeding instructions available for the infants not on table foods.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The preschool classroom was over ratio by 3 children. Note: A second caregiver arrived during my inspection.

746.2207(a) - Screen Time Activities - Not Used for Under Two

The tv was on in the infant room. Note: The caregiver was informed that the tv cannot be on for children under 2 years old and it was shut off.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

In the infant 2 class has a play pen that has the net part ripped, the air vent has visible dirt on it, the paint is chipping. The table in the pre-k room is broken and laying on it's side. There are three broken shelves in the pre-k room as well. In the toddler 2 class there is a ceiling tile that is saturated and appears to have mold on it.

The director has been absent for an extended period of time and has not requalified a director to take over in their absence.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

There is a merry go round, on the playground, that does not have loose fill in the fall zones of the equipment.

746.801(1) - Required Records Maintained and Made Available - Children's Records

Children records were not available during my inspection due to the person in charge not having access to the office.

746.4803 - Measurement of Use Zone for Stationary Equipment

The arch climber, on the playground, does not have the required 6 feet of use zone.

746.1013(a)(3) - Director's Absence - Caregiver in Charge Has Access to All Records

The caregiver in charge did not have access to the files and paperwork needed for the inspection.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

The infant room teachers were unaware of each child's age to determine the specified age group for ratio. Note: The director went into the room and ensured the staff had access to each child's age so they could determine proper ratios.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The toddler room was over ratio by 4 children. One caregiver was supervising 15 children with the specified age group being 2 years old. Ratio for 2 year olds is s1 caregiver to 11 children.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Caregivers admit to being on their phone during the day. The director admits to having a problem with caregivers being on their phone.

Staff files were not available for review at the operation during my inspection.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

The straps in the infant table were not being used while the infants were sitting in the seats eating lunch. Note: The caregiver strapped the children in.

Five children files had a parent listed as their emergency contact.

746.611(a)(1) - Required Health Statement - Written, dated within past year, indicates child is physically able to participate in operation's child-care program

Four children files did not have a health statement.
