Based on the evidence gathered, a child with known special care needs and medication requirements did not have a special care needs statement in their admissions file to inform staff how they can best support the child.

Based on the evidence gathered, a caregiver used prohibited punishment when they grabbed and pulled a child by the arms to take the child into the front office because the child did not follow directions.

Several gallon jugs of bleach were observed in the children's restroom and accessible to children. This was corrected when it was addressed with the person in charge and the bottles were removed from the restroom.

Based on the evidence gathered, infant caregivers did not follow written feeding instructions from the parent of an 8-month-old, between the following dates: 9/18/24 through 9/30/24.
