Staff files evaluated were incomplete and missing the following documentation: - Photo ID - High School Diploma or Equivalent - Notarized Licensing Affidavit and Pre-Employment Affidavit

Documentation reviewed showed the 24 hours of annual training for 10 staff have not been completed.

Documentation showing 1 caregiver had completed CPR and First Aid Training was not available during the inspection. Note- Documentation was submitted showing the caregiver completed the training on 11/1/2024.

Documentation reviewed showed the director is missing 2 hours of annual trainings.
