746.3701 - Safety - Areas Free From Hazards

The climbing piece of equipment on the playground area was observed to be unsteady. A crack was observed in the plastic slide located on the playground that the children had access to.

746.3415(4) - Employee Handwashing After Diapering a Child

A caregiver was observed helping toilet a child while wearing a pair of the gloves. The caregiver was observed sanitizing nap mats without washing their hands and switching out with a new pair of gloves.

746.3407 - Maintenance of Building, Grounds and Equipment

During the inspection there was one trashcan observed without a lid and accessible to children. A toilet was observed with urine in it in a classroom being used by the children in care. There was chipped paint observed throughout the facility, cracked floors and floors with missing pieces were observed throughout the facility. A hole was observed in the wall of the last room in the facility.

746.5401 - Requirement of Gas Leak Inspection

The operation does not have a current gas test. The last gas test was conducted on 02/09/24.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

During the inspection, a caregiver was observed laying down on their personal phone with a group of children.

746.3313(1) - Daily Menus - Record Comparable Substitutions

During the inspection, it was found that the operation does not always follow the daily menu. When changes are made to the menu, it is not documented.

746.2415(a)(1) - Prohibited Equipment for Infants - Baby Walkers

A baby walker was observed in the infant classroom. This was corrected at the inspection when the walker was removed from the classroom.
