A second classroom was observed during nap time ratios, where a child was observed under the age group. This was corrected when a second classroom combined and the child was moved to another classroom bringing the room into compliance with 10 children and two caregivers.

A child was observed asleep in a restrictive device. The child was removed from the device and placed on the floor. Another child was observed sleeping face down on the stomach.

Two classrooms were observed out of ratio. The toddler room was out of ratio by 1 child, while one caregiver was on break. In the three-year-old classroom, one caregiver was caring for 18 children during naptime, bringing the room out of ratio by 3 children. Another incident was observed when a caregiver stepped out of the room, leaving one caregiver with 22 children.

5 cribs was observed with loose fitted sheets. Two of the cribs had children asleep in them. The mattresses did not fit the crib base per manufacturing instructions.
