At the time of the inspection an asthma inhaler was present for a child that was expired.

744.3817(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

At the time of inspection one of the transportation buses did not have a fire extinguisher as required.

744.1311(a) - Operation and Program Director Annual Training 20 Hours Required

At the time of inspection, the director did not have current training certificates available for review. Trainig certificates for 2025 had not been printed and were not accessible electronically. The operation will forward all certificates via email. Training certificates were forwarded for the director.

744.1309(a)(1) - Documented Annual Training-15 Hours Required Caregiver

At the time of inspection, current training certificates for staff were not immediately accessible via the electronic system. The operation will access the training certificates and forward via email. Training certificates were not provided for staff.

744.2669 - Food Allergy Plan-Emergency plan for diagnosed child, signed by health care professional and parent

At the time of inspection, one child with a diagnosed food allergy did not have an emergency food allergy plan available.

744.2655(c)(4) - Medication Records - Name of Employee Who Administered

At the time of inspection the medication log was not documented with the required informaiton. The single log entry (6/25/25) did not include the time, name or amount of the medication or the name of the person who administered it. This was corrected at inspection by staff completing the log to include all required information.

744.3805(3) - Safety Precautions for Loading and Unloading Children - Account for All Children

It was determined that the driver did not account for all children at the time of unloading which resulted in a child being left on the vehicle.
