Through the review of video surveillance, a caregiver was observed open handed hitting a child on their arm in an effort to discipline the child.

746.2803(4)(D) - Positive Methods of Discipline and Guidance - Brief Supervised Separation

Through the review of video surveillance, it was determined that a 2-year-old child was made to sit in time out for a period of 16 minutes.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

Through the review a video surveillance a caregiver was observed using their personal cell phone to take selfies, text and watch videos while supervising children in care.

746.2905(1) - Children Not Required to Sleep - May Not Force Child to Sleep

Through the review of video surveillance, it was determined that a caregiver forced a child to lay down to rest by pushing down on the child's chest in an effort to make the child lay on their mat.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It was determined through the course of the investigation that a child in care was left unattended on the playground.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

A 4 month old was observed sleeping in the crib on a boppy pillow. This was corrected at inspection when the boppy pillow was removed from the crib bringing the operation into compliance.

It was determined through the course of the investigation that the infant room has not maintained proper ratio.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An 6 month old infant was observed sleeping in a swing. This was corrected at inspection when the infant was moved to their crib bringing the operation into compliance.

746.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

It was determined through video surveillance that a caregiver in the infant room was using their cell phone for personal use while supervising infants alone.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

2 caregiver files reviewed, that have been employed longer than 90 days, did not have a current CPR certification.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The operation had not conducted a fire drill since July 2025.This was corrected at inspection when the operation conducted a fire drill bringing the operation into compliance.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

2 infants in the infant room were not buckled into the kidney bean toddler table. This was corrected at inspection when the children were buckled in.
