Two bottles in Classroom 2 were observed to not be labeled. This was corrected at inspection when the caregiver labeled the bottles.

A child was left in the bathroom while the class transitioned to breakfast.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A child was left for 3-5 minutes unsupervised outside the main building.

746.3301(h) - Nutrition and Food Service - Food not a reward

A caregiver used M&Ms as an award for the children to listen and moved onto the carpet. This was corrected by reviewing the importance of nurition for children.

746.701(5) - Licensing Incident/Illness Report Form - Situations Placing Children at Risk

There was a child who was pulled by the wrist across the playground and the parent did not receive an incident report.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

There was a caregiver who was witnessed grabbing a child by their wrist and pulling them across the playground.

746.305(a)(6) - Report Situation Placing Children at Risk

The operation did not self-report to childcare regulations after parents expressed concerns about how a caregiver treated their child.
