745.651(1) - Background Check Validation - You must inactivate a subject who is no longer associated with your operation within 7 days of leaving

Based on information obtained during the investigation, it was determined that an employee who left in October 2025 was not inactivated until February 2026.

745.619 - Background Check Requirements - Submit All Identifying Information

Based on information obtained during the investigation, it was determined that email address for the individual was incorrectly entered and the email address for the operation was used. This resulted in accidental disclosure of personal information.

746.201(6) - Background Checks Kept Confidential and Not Disclosed to Unauthorized Persons

Based on information obtained during the course of the investigation, it has been determined that the background check and personal confidential information was disclosed to unauthorized persons. The operation no longer was authorized to use this information after the individual left employment. The background check information was accessed without the individual's consent and was processed by the operation three times after employment ended.

746.605(6) - Required Admission Information - Emergency Contact

Of 10 children's files reviewed, 3 did not have addresses for the emergency contact.

746.605(12) - Required Admission Information - Special Needs Statement

10 children were missing special care needs information in their admission paperwork.

746.901(5) - Required Personnel Records - Required Affidavit (Form 2912)

3 caregivers were missing the pre-employment affidavit in their file and these staff were hired after the change took effect on 09.01.2023. This was corrected at inspection.
