The operation has not verified liability insurance. Verification was due by 09/18/2025.

The operation does not have a current fire inspection on file, the last fire inspectioin was conducted in February 2024.

Four of the eight children's record reviewed were missing preschool health statements.

One of the eight staff records reviewed were missing ten clock hours of annual training.

Four of the eight staff records reviewed do not have annual training hours in handling, storing, and disposing of hazardous materials. One of the four staff members is also missing annual training in preventing and responding to food allergies.

two of the ten children's files reviewed did not have the child's immunization record on file.

746.5625(b) - Electronic Child Safety Alarm - installed and maintained according to manufacturer's instructions

One of the vans used to transport children does not have a child safety alarm installed.

746.5607(a) - Child Passenger Safety Seat System - Must Use for Each Child in When Transporting and be Properly Installed

Four of ten safety belts in the van are not in working condition, they do not latch. The operation is not currently using those seats to transport children.

746.2805 - Prohibited Punishments - No Harsh, Cruel or Unusual

Based on the investigation findings, this standard was reviewed and found to be in non-compliance with The Minimum Standards for Child-Care Centers. Children at the center stated that an operational staff member hits the children on the hand as a form of discipline.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the investigation findings, this standard was reviewed and found to be in non-compliance with The Minimum Standards for Child-Care Centers. Children in the school age classroom were unsupervised on at least one occasion.
