A fire inspection had not been conducted. The last fire inspection for previous owners was on 10/17/2024.

Seven out of 10 preschool children's records reviewed, the health statement from health professionals were not available during the inspection.

746.3407 - Maintenance of Building, Grounds and Equipment

1/There were stained tiles on ceilings in the after-school classroom. Note: corrected. The center had materials and replaced stained tiles on 05/05/2025. 2/There was one missing toilet seat in one of the restrooms in after-school classroom which was closed temporarily. Note: corrected. The center had brand new toilet seat and installed it on 05/05/2025. Pictures taken before and after fixings.

746.5531 - Child-Care Center Must be Equipped with a Working Carbon Monoxide (CO) Detection System

The Carbon Monoxide (CO) Detection did not work when the inspector tested it at inspection. Note: corrected. The daycare installed the brand-new Carbon Monoxide (CO) Detector, and it worked at inspection.

There were 4 out of 8 providers did not have current FA certificate of file. Note: corrected. The director sent current FA certificates for 4 providers as required.

There were 4 out of 8 providers did not have current CPR certificate of file. Note: corrected. The director sent current CPR certificates for 4 providers as required.

746.1205(a)(1) - Responsibilities of Caregivers - Know children responsible for

One provider did not know the age of one toddler in care and one provider in charge did not know the total number of children in care. Note: corrected. The inspector asked and rechecked another time during the investigation. Both providers answered the questions correctly.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The daycare did not practice fire drill monthly for 01/2025. Note: corrected. The director sent their practice fire drill monthly to the inspector. The practice drill was conducted on 02/26/2025.

746.3417(3) - Children Handwashing After Toileting or Diapering

There were 2 out of 5 toddlers did not wash their hands after diapers changing. Note: corrected. The caregiver washed their hands after noticing that they have not washed their hands at inspection. The inspector rechecked another time during the investigation, the caregiver washed their hands after diapers changing.

746.5625(a) - Electronic Child Safety Alarm - is equipped in each vehicle

There was 1 vehicle that was purchased after 12/31/2013 and is a 13 seater. The vehicle does not have the electronic safety alarm installed.

746.5617(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

There were two transportation vehicles that were not equipped with working fire extinguishers. One indicated it was overcharged and the other indicated it was empty. Note: The fire extinguishers were replaced in the two transportation vehicles during the inspection.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

The director did not have a current CPR and First aid training. The last training has expired on 10/1/2024.
