746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

One child with food allergies has an Epi-pen in case of emergency. The Epi-pen was observed and the Food Allergey Emergency Plan (FARE form) is not on file.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The annual fire inspection report was observed during the inspection and expired on 6/26/2025.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

Two caregivers who had previously received "eligible" notifications on their background check had been changed from active employment to inactive employment status. They are currently employed and were present at the operation during the unannounced inspection and did not have a current background check. This was corrected during the inspection. The caregivers cannot be present until the background check results have been received.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It has been determined that a child was left unattended for approximately two minutes while the rest of the class went outside.

746.901 - Required Personnel Records

Ten employee files were reviewed during the inspection and eight were observed to be incomplete.

746.1301(a)(1)(A) - Training Requirements for Employees, Caregivers, and Directors - Orientation

Three of the employee training records that were reviewed during the inspection were incomplete.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

Five bus driver files were reviewed and did not have the 2 hours of transportation training. Employees who transport children are required to have 2 hours of transportation training prior to transporting children and each year thereafter.
