Information obtained through interviews and observations indicated the caregiver did not demonstrate self-control when physically moving a child in a frustrating and inappropriate manner.

Classroom wall was not in good repair with a whole and exposed sheetrock. Walls appeared very worn.

The operation failed to complete/document an incident report after being notified of the medical diagnosis.

A caregiver failed to demonstrate good judgment. The caregiver was aware that a child had a food allergy that required an epi pen and failed to inform management. The caregiver was also aware that the epi pen was stored in the child's backpack which was retrieved by a child in care.

A caregiver was not aware that a child obtained an Epi Pen from another child's backpack while in care.

The director was not aware of a child in care with a diagnosed food allergy requiring an epi pen. The child had a food allergy plan on file at the operation.

A child in care retrieved an epi pen from another child's backpack and stuck themselves with the epi pen's injection point.
