There is not adequate loose fill of at least six inches around one of the slides on the playground.

746.1301(a)(4)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric CPR

One of the staff files reviewed was not up to date on CPR and first aid training. It expired in April of 2026.

746.2429 - Covering Infant's Head or Crib

Evidence obtained during the investigation shows that infants heads were being covered while they were sleeping in cribs.

Based on the evidence obtained during the investigation, a caregiver did not use good judgement or self-control when they forced a child to eat from a bottle.

Based on the evidence obtained during an investigation, a caregiver was physically rough with children while caring for them.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

A classroom with children younger than 18 months in it had four children over the allowed ratio of 5 children per one caregiver.
