Three of ten children's records audited did not have current immunizations and two did not have current health statements.

Hand soap and paper towels were not independently accessible to children at the Preschool hand sinks. This was corrected at inspection when a soap dispenser was provided and hand towels were stocked.

The loose-fill under the play structures on the Pre-K playground and the dual slides in the Toddler playground were less than six-inches.

Upon review of video footage provided, it was observed that an infant was placed in their crib awake and remained awake in the crib for over 30 minutes.

Upon review of video footage provided, a caregiver was observed picking up an approximately 7-week-old infant out of their crib, and walking across the room, failing to support the infant's neck.

At time of inspection, a prek caregiver stated they had twelve children but class had thirteen. A time of inspection a early preschool caregiver stated to have nine children but class had eight.

At time of inspection a prek caregiver was observed yelling in a childs face.

At time of inspection a PreKinder caregiver did not know the number of children they were caring for. At time of inspection a PreKinder caregiver did not know the ages of children caring for.

At time of inspection a diaper changing station in a preschool class was observed to have debris. At time of inspection multiple handwashing sinks were observed to be stained. At time of inspection a childrens restroom door was observed to have a hole at the bottom of the door.

Based on information gathered, a child was left on the playground unsupervised.

Based on information gathered, caregiver did not interact with child in a positive manner, when caregiver lifted child by one arm and pulled child to the other side of caregiver.
