The operation failed to notify the parents of a situation that placed their child at risk immediately, as required by minimum standards. The parents were notified of the incident at the time of pick-up, several hours after the incident occurred.

During the course of the investigation, it was confirmed that a child was left unsupervised on the playground for approximately less than one minute.

One of the four staff files was missing 2023-2024 training on handling and storing hazard materials.

One of the four staff files was missing training in recognizing and preventing shaken baby syndrome and abusive head trauma, Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) and understanding early childhood brain development.
