744.1301(a)(3)(A) - Training Requirements for Employees - Pediatric First Aid with Rescue Breathing

Caregiver at operation inspection on 5/20/26 did not have up to date first aid training available for review. Caregiver was observed having unsupervised access to children in care.

744.1301(a)(1)(A) - Training Requirements for Employees - Orientation

Two caregivers present at time of inspection did not have a complete operation specific orientation available to review at time of inspection. This was corrected at time of inspection when director conducted a orientation and documentation was shared with CCR.

744.901 - Required Personnel Records

The operation was unable to provide employee records for evaluation during hours of operation.

744.303(4) - Report Change in Director

The site director resigned in December of 2024. The operation failed to report the change to CCR.

744.2659 - Medication No Longer Used

The operation had expired prescription medication that was prescribed to a child no longer in care stored on site.

744.1321(b) - Block certification training include hours and topics

2 block training staff certificates reviewed did not specify the time spent for each topic of training.

744.1607 - Maximum Group Size

Two caregivers had 39 children in care, ages 4 to 10 years old, which exceeded the maximum group size of 35.

744.301(6) - Notification of Change in Hours, Days, Months of Operation

The operation provided care for children outside of their listed hours of operation on 10/15/24 and did not notify Child Care Regulation as required. The director stated that this school year, the after school program is providing care during some school holidays, such as student holidays or teacher work days. This was corrected at inspection by the director notifying of the changes and a school calendar being provided.
