The bus driver did not make a good judgment by entering the intersection before all vehicles had cleared, resulting in a car accident.

The bus driver who transported the children to and from school did not have an official copy of the CPR and First Aid certificate showing the training date and expiration date.

During the inspection, CCR observed a school-age child and a Pre-K child leaving the restroom unsupervised while the caregivers remained in the classroom.

There is sufficient evidence to support that three caregivers placed children in highchairs as a form of discipline.

CCR reviewed a file for a school-age child and observed that the child's school information was missing.

The operation playground has two stationary equipment. One equipment with a slide is located three and a half feet away from the swings' use zone, while a playhouse is four and a half feet away from the other stationary equipment.

The Second director received 0 of the 30 clock hours of required annual training 10/24/2023-10/24/2024 training year. The director will need to receive 30 clock hours of make-up training that will not count towards the current training year.

It was observed in the pre-school classroom a caregivers supervising 15 children specified age group three years old while they took a nap. The caregiver had conditions they were not allowed to be alone with children.

In the toddler classroom they was not adequate light when children were napping, the caregiver had to use a flash light to see the children's face. They was a lap on, it was on the other side of the classroom.

A three month old infant was swaddled in a bouncer. Corrected: When the director took the blanket off the infant.

746.2426(a)(2) - Infant May Not Sleep in a Restrictive Device - Remove the infant from the device and place the infant in a crib as soon as possible

A 8 and 3 month old child were sleep in a swing the caregiver did not remove them when they fell asleep they continue to push the swing. Corrected: When director had caregiver remove children from the swing and put them in the crib.
