Two orbital spinners on the school age playground did not have any loose-fill or unitary surfacing under them.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

It was determined that a child was left unsupervised for approximately three minutes.

746.3501(1) - Diaper Changing Steps- Prompt

During the investigation, it was determined that several children were not promptly changed when they had soiled or wet diapers/clothing.

746.701(5) - Licensing Incident/Illness Report Form - Situations Placing Children at Risk

An incident report was not provided to the parent when a non-routine situation that placed, or may have placed, a child at risk for injury or harm when a child was able to walk down the side of the gated corridor unsupervised to unlock the latch and open the exit metal gate that leads to the parking lot.

At time of inspection, a PreK caregiver stated class had twenty children when they had nineteen. This was corrected at inspection.

At time of inspection, cleaning chemicals were observed at child's reach in the school aged class.

746.3407 - Maintenance of Building, Grounds and Equipment

At time of inspection the grass to the school aged playground was approxiamelty 4 inches tall. At time of inspection there was a gap in the school aged fence approximatley 3 inches.

746.5105 - Corrections Made to Fire Inspection

At time of inspection, operations last fire inspection was conducted on 11.4.24 that required corrections made and have not been completed.

746.4503(a) - Individual Cot, Bed, or Mat for Each Walking Child Through 4 Years of Age

At time of inspection, children were not sleeping in their assigned cot in two different toddler classes .

746.901(7)(A) - Required Personnel Records - signed dated statement that employee received operational policies

At time of inspection, ten staff files did not have verification of acknowledgment of staff and operational policies.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

At the time of inspection, the director and drivers had completed only one hour of transportation training, which is below the required two hours.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

At time of inspection one out of ten staff files had an expired CPR/First Aid certification.

746.3607(2) - Critical Injury - Give First Aid or CPR

Throughout the investigation, it was noted that caregivers did not assess the child when they were crying and holding their arm.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Caregivers exhibited lack of judgement by failing to notify the parents and their administration of a child that exhibited and expressed signs of being injured. The child had been intermittently crying for from 850AM until 5PM . The child was later diagnosed with a dislocated and fractured finger.

746.307(b)(1) - AP Parental Communication - Injury Requires Treatment by Health Care Professional or Hospitalization

Throughout the investigation, it was noted that a toddler displayed signs of a possible injured finger. The child cried intermittently through out the day about their finger. The caregivers did not notify the parent of the injury. The child had a dislocated and fractured finger.

746.1309(e) - Documented Annual Training- Required Content

Three out of ten staff files reviewed were missing training over handling and storing of hazardous materials training.
