746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

Two infants ages 9 months old and 15 months old were observed being combined with the older children ages 3-4 years old during the inspection. This was corrected when the infants were brought back to their classroom during the inspection.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

According to the information gathered from the DFPS investigation, supervision was not adequately maintained when a child exited the building without the caregiver's knowledge.

746.305(a)(6) - Report Situation Placing Children at Risk

Based on the information obtained from the investigation, the operation failed to report an incident to the CCR when a child exited the building without the caregiver's knowledge.

746.631(a) - Sign-in and Sign-out Tracking System

Based on the information obtained from the DFPS investigation, it is determined that the attendance records for the children in care was not being maintained and documented properly.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Based on the information obtained from the DFPS investigation, it is determined that the caregivers at the operation did not implement good judgment or self-control when handling children in care.

746.2805 - Prohibited Punishments - No Harsh, Cruel or Unusual

Based on the information obtained from the DFPS investigation, it is determined that the owner of the operation spanked a child in care, and another caregiver was confirmed to occasionally flick the children's forehead while in care as a form of discipline.

746.305(a)(6) - Report Situation Placing Children at Risk

Based on the information obtained from the investigation, it is determined that the operation failed to report to Child Care Regulation when a caregiver inappropriately disciplined a child in care.
